To add a Health Care Agent, click the Name a Health Care Agent option in the left-hand navigation menu.
On the Name Your Primary Health Care Agent page, enter the Health Care Agent’s information, specify how the patient wants their information shared or not shared, and select which powers (some or all) the patient wishes to grant.
To limit the powers assigned to a Health Care Agent, click I want to limit the powers they have and toggle the desired powers according to the patient’s preferences.
Once you have filled in all the information and desired selections, click the blue Save button. The Name a Health Care Agent section will now display the updated number of created/saved Health Care Agents.
If the patient has opted to notify the individual that they have selected them as a Health Care Agent, the individual will receive an email or SMS notice at the contact information provided.
